Delta Dental PPO Plus Premier — Indemnity PPO

This plan offers flexibility with an extensive network and the ability to see any licensed dentist, even out of state or internationally. 

Key features: 

• $2,000 annual benefit per person, excluding preventive care. 
• $1,500-lifetime max for orthodontia.
• No referrals necessary — see any dentist. 
• More than 85% of Arizona dentists are in-network. 
• Out-of-network services are covered at reduced rates.

UnitedHealthcare Solstice S800B — Prepaid DHMO 

This plan focuses on preventive care and offers lower monthly premiums. You must choose a participating dentist to coordinate your care. 

Key features: 

• No deductible or annual maximums. 
• No claim forms. 
• Fixed copayments for services. 

• Set lab fees for prosthodontic materials.
