How to use the Advantage Program

Visit the Avesis website to find a provider. Using in-network providers allows you to maximize your vision care benefit. Schedule an appointment with your chosen provider for services and identify yourself as an Avesis member employed by the state of Arizona.

Out-of-network benefits and claims 

If you receive services from a non-participating provider, you will pay the provider in full at the time of service and submit a claim to Avesis for reimbursement. Send the out-of-network claim form and itemized receipt to Avesis within three months of the service date to be eligible for reimbursement. You can obtain and download the claim form from the Avesis website.
